How to create, edit, send, and review completed forms in OS for Health Business

OS for Health Business enables you to conveniently create and send intake, consent, and assessment forms within the same tool you use to manage the rest of your client care experience. You no longer have to manage multiple tools, and coordinate entering form responses from one platform into the client health record.


Creating a New Form

  1. To create a new form, navigate to the "Forms" tab in your left menu.

  2. Select "New Form" at the top right.

  3. This will open the form creation flow. From there, the form builder will open. On the left, you'll have options for various field types on the left, and form settings on the right.

  4. When creating fields, you'll have the option to map them to key elements of the client record, such as First Name, Last Name, and Date of Birth. This prevents you and your client from having to enter the same details multiple times.

  5. Enter and edit your desired fields, and then you can save your draft, preview, or publish your form. Once published, your form will be available to send to clients.


Sending a Form

  1. From the Forms tab, select the form you'd like to send to your client(s)

  2. At the top right, select the "send form" button

  3. From there, you'll be able to search for and select the clients you'd like to send the form to

  4. After selecting your desired client(s), choose "Send to Client" at the bottom right to send your form

  5. After sending, you'll be able to track the progress of individual clients toward form completion, as well as overall metrics for the form.


The Form Summary Page

For forms you've already created, you have several options available from the form summary page. You can send your form, make edits to your form, view completed form responses, or send reminders to clients who have yet to complete assigned forms.

The three dots available at the top right provide options to duplicate or archive your form.


Reviewing Form Responses

Once a client has completed their assigned form, you'll be able to select "View" at the end of their row to review their responses. You'll have the option to make notes within the response viewer as needed.
